How do I know what X/Y coordinates to use?

Coordinates are measured in points from the bottom-left corner of the page; it usually takes a little trial and error — try a rough guess, check the result, and adjust the numbers until the box lines up.

Is the redacted content actually removed, or just covered?

The black rectangle is drawn as a new shape on top of the page content — the underlying text or image data is not deleted from the file itself, only visually covered, so this is best used together with Compress PDF's re-save step, or for visual-only redaction rather than forensic-grade removal.
